Eligibility Requirements

Patients are eligible for Triple Aim Foundation Assistance if:

Reside in the US

The patient resides and is receiving treatment in the United States or U.S. territories. (U.S. citizenship is not a requirement)

Are covered by Medicare Part D

The patient must be covered by Medicare Part D. This program is aimed at lowering patient financial toxicity.

Have a valid prescription

The patient must have a valid prescription for a course of immunology provider-administered medications for treatment of a documented medical condition.

Meet income requirements

The patient’s annual income must be below 500% of the Federal Poverty Line (FPL). The U.S. Federal Poverty Guidelines change every year; current guidelines are available here.
